Referrals can be made by Early Intervention Providers, Primary Care Physicians and Parents/Guardians.
Intake visit
At this first appointment we will meet virtually to complete an intake questionnaire and review past evaluations, early intervention or school district assessment and request such reports.
It is not necessary that you have the documents at hand, nor that your child has been previously evaluated for this visit.
It is recommended that you have on hand the phone number or email of the child’s closest teachers, family and friends, since it is necessary for them to fill out an observation form, to understand how the child relates to others.
Initial developmental evaluation
This meeting should be scheduled at the time of day when your child is most playful and rested, in a place where s/he feels the most comfortable.
The initial developmental evaluation will be scheduled after the intake.
Dr Gomez, will assess social communication and restrictive, repetitive behavior using an age appropriate systematic observation screening test. Parents are expected to fill out a DSM-V interview questionnaire during this visit. Generally, a detailed intake, parents interview and a systemic observation test is enough to diagnose Autism Spectrum Disorder, if this is the case, we will discuss the diagnosis and create a comprehensive treatment plan balancing risks, benefits and the quality of life that you hope for your child and your family. If you would like to schedule a separate appointment to discuss the results, that can be arranged.
A complete written report will be available after 48hrs.
If your child does not meet the diagnostic criteria for Autism but we find other related behavioral or mental health conditions we will create a treatment plan accordingly.
ADOS-2 Assessment
If after conducting the systematic observation screening test, Dr Gomez is unable to get a clear picture of deficiencies in social communication and restrictive, repetitive behaviors, but documentation is suggestive of Autism Spectrum Disorder, then, an Autism Diagnostic Observation Schedule 2nd edition (ADOS-2), will be scheduled. This is a semi-structured, standardized assessment instrument that includes a number of play-based activities designed to obtain information in behaviors that are typical of Autism. During this visit we will review all documentation that we have collected so far and the result of the ADOS-2 report after coding it. If an Autism Spectrum Disorder is found, a comprehensive treatment plan will be created with the family. If you would like to schedule a separate visit to discuss results, that can be arranged as well.
Not all cases of ASD need this assessment and will only be done if a clear diagnosis could not be reached on previous evaluation.
For this evaluation we will meet either at Tiny Notes in Port Orchard or in the offices of Poulsbo Parks and Rec in Poulsbo.
If after Medical and Developmental records are reviewed and ADOS-2 assessment is completed a diagnosis of Autism Spectrum Disorder is not clear, then recommendations for further evaluations will be provided to parents to share with primary care provider for proper referrals and community support.
Some complex developmental cases may be a better fit for our regional autism centers at Mary Bridge or Seattle Children's.
This would include:
- Children with complicated genetic conditions.
- Children with multiple diagnoses, like Oppositional defiant disorder, Conduct disorder, Disruptive mood dysregulation disorder, etc.
- Bipolar disorder.
- Severe Anxiety/Depression not responsive to treatment.
- Extensive trauma.
- Families requesting a “rule-out”.
- Children who have had previous evaluations for autism.
